Menu Path: Business Administration >> Survey Templates Configuration
1. Click on the Create New button to create a new Survey Template.
2. In the General Information fill in the required details.
Field Description

| Template Name | Name of the Template | 
| Description | Description of the Survey Template | 
| Owner | Auto Populated with the logged-in User’s name and ID | 
| Execution Schedule | Manual Execution Schedule | 
| Respondent Visibility | Anonymous- Both the Supplier and the User from the Supplier remains anonymous | 
3. Click on the Add Question button to add questions to the Survey Template.
4. In the Pop up window that appears, fill in the details and click on Save.

| Question Type | Choose a Question Type from the list | 
| Answer Type | The Type of answers of the detailed question can be | 
| Mandatory | Select to make this question mandatory in the Survey Form, without filling which the Supplier will not be able to Submit the Survey Form. | 
| Comments | Choose Comments to let the Supplier enter some commets for the question asked | 
| Attachments | Provision to attach documents by the Supplier to substantiate the question. | 
5. You are further provided with the provision to edit/delete questions that have been created.
6. Click on the Save button to Create a Survey Template.
